1. Download and save our questionnaire. You will need to save it once it is completed and send it as an attachment to us with a photo of you and your partner.

2. You can email the completed questionnaire with your photo to
info@themoneytest.com or fax us at (204) 949-0050.

You can also mail your questionnaire and photograph and/or a home video of you and your partner. We can accept any format other than Hi-8 tapes.
